A prospective study of transmission of Multidrug-Resistant Organisms (MDROs) between environmental sites and hospitalized patients—the TransFER study

Objective
Hospital environmental surfaces are frequently contaminated by microorganisms. However, the causal mechanism of bacterial contamination of the environment as a source of transmission is still debated. This prospective study was performed to characterize the nature of multidrug-resistant organism (MDRO) transmission between the environment and patients using standard microbiological and molecular techniques.
SettingProspective cohort study at 2 academic medical centers.
DesignA prospective multicenter study to characterize the nature of bacterial transfer events between patients and environmental surfaces in rooms that previously housed patients with 1 of 4 ‘marker’ MDROs: methicillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci, Clostridium difficile, and MDR Acinetobacter baumannii. Environmental and patient microbiological samples were obtained on admission into a freshly disinfected inpatient room. Repeat samples from room surfaces and patients were taken on days 3 and 7 and each week the patient stayed in the same room. The bacterial identity, antibiotic susceptibility, and molecular sequences were compared between organisms found in the environment samples and patient sources.
ResultsWe enrolled 80 patient–room admissions; 9 of these patients (11.3%) were asymptomatically colonized with MDROs at study entry. Hospital room surfaces were contaminated with MDROs despite terminal disinfection in 44 cases (55%). Microbiological Bacterial Transfer events either to the patient, the environment, or both occurred in 12 patient encounters (18.5%) from the microbiologically evaluable cohort.
ConclusionsMicrobiological Bacterial Transfer events between patients and the environment were observed in 18.5% of patient encounters and occurred early in the admission. This study suggests that research on prevention methods beyond the standard practice of room disinfection at the end of a patient’s stay is needed to better prevent acquisition of MDROs through the environment.

This study compared the life histories of Hemidactylus frenatus, a significant invasive gecko, and Phyllodactylus palmeus, a Honduran endemic, over 10 wk, June–August 2013 at 12 study sites on the Honduran island of Cayo Menor of the Cayo Cochinos archipelago where H. frenatus arrived in 2008. Three different life-history traits related to invasion success were measured: body size, fecundity and population size. During the study 140 natives and 37 non-natives were captured, weighed, measured and marked uniquely. The number of gravid females and number of eggs were also recorded. Phyllodactylus palmeus was the significantly larger of the two species (60% larger mass, 25% longer SVL) and had higher population abundance at all 12 study sites with some sites yielding no H. frenatus individuals. However, H. frenatus had a larger proportion of gravid females. Observations that the native species is more common despite being sympatric with a known aggressive invader suggest two possibilities: the island is at the start of an invasion, or that the two species co-exist in a more stable fashion.
Domain Structure of a Unique Bacterial Red Light Photoreceptor as Revealed by Atomic Force Microscopy

Bacteriophytochromes (BphPs) are red-light photoreceptors found in photosynthetic and nonphotosynthetic bacteria that have been recently engineered as infrared fluorescent tissue markers. Light-induced, global structural changes are proposed to originate within their covalently bound biliverdin chromophore and propagate through the protein. Classical BphPs undergo reversible photoconversion between spectrally distinct light absorbing states, red (Pr) and far-red (Pfr), respectively. RpBph3 (P3), from Rhodopseudomonas palustris, photoconverts between a Pr and a unique near-red (Pnr) light-absorbing state. Due to size and photosensitivity of BphPs, structures of the intact proteins have not been resolved by nuclear magnetic resonance and/or X-ray crystallography. Therefore, structural details about the light and dark-adapted structures of the intact BphPs are not well understood at the molecular level. We have utilized fluid cell atomic force microscopy (AFM) to investigate the domain structure of intact P3 in its light-adapted state (Pnr). By varying the concentration of the protein, deposition time, and the ionic strength of the buffer, the aggregation of P3 on a mica surface can be controlled and single dimers may be observed in a biologically relevant media. Domain resolution has been achieved for several orientations of the dimer on the surface. The structural dimensions of the dimer have been compared to a modeled BphP in its intact form generated using PyMOL software. AFM experiments are currently underway to analyze the dark-adapted state (Pr) of P3 in order to observe the anticipated structural changes. Ultimately, the goal is to use AFM and other surface analytical methods such as scanning tunneling microscopy and electron microscopy to gain new insight into the unique photochemistry of P3.

DNA polymorphism and host range in the take-all fungus, Gaeumannomyces graminis

Thirty-five isolates of Gaeumannomyces graminis were tested for ability to infect wheat, rye and oats, and for DNA polymorphisms using nuclear ribosomal DNA (rDNA) RFLP patterns and RAPD analysis. In general, the cereal-attacking isolates could be readily assigned to the rye-attacking (R) or non-rye-attacking (N) subgroups of var. tritici, or to var. avenae, on the basis of either of these molecular approaches. A small number of isolates gave anomalous rDNA RFLP patterns, but could nevertheless be assigned to one of the three groups by RAPD analysis. Two related G. graminis var. tritici isolates (T1–1 and T1–2) clearly grouped with the N rather than the R var. tritici subgroup on the basis of molecular analysis but were pathogenic to rye, indicating that the ability to infect this host may have arisen more than once. An earlier phylogenetic study of Gaeumannomyces involving DNA sequence analysis of the internal transcribed spacers of the rDNA indicated that several oat-infecting isolates originally classified as G. graminis var. tritici could be grouped with var. avenae isolates. From the rDNA RFLP and RAPD analysis described here, however, these isolates appear to be intermediate between var. avenae and var. tritici, although it is not clear whether they represent evidence of inter-varietal sexual hybridization.
